Are inhaled long-acting Beta-agonists (laba) really harmful in adult asthmatics?

چکیده
Mr. SS, a 32 year old Chinese male, is a known case of moderately severe persistent asthma since the past 17 years. He is a palm oil plantation worker for the past 6 years. He is a nonsmoker, non-hypertensive and has no history of diabetes mellitus. There is a family history of bronchial asthma in both his mother and maternal grandfather. On examination, the patient appears tachypnoeic, with a respiratory rate of 30 breaths per minute. His pulse rate is 110 beats per minute. Peak expiratory flow done at the time of clinical examination is 200 L/min. He is currently on a combination of fluticasone propionate (250 mcg) and salmeterol (50 mcg) administered by an inhaler device twice a day. He has heard that the use of inhaled long-acting beta agonists (LABA) is considered controversial and potentially damaging and is therefore concerned about further usage of this therapy.

Observational study of the additive effects of pranlukast on inflammatory markers of clinically stable asthma with inhaled corticosteroids and long-acting beta 2 agonists.
BACKGROUND Little is understood about the additive effects of leukotriene receptor antagonists (LTRA) on asthmatics currently medicated with inhaled corticosteroids (ICS) and long-acting beta(2)-agonists (LABA). OBJECTIVES The present study examines the anti-inflammatory effects of the LTRA pranlukast in addition to ICS and LABA, among asthmatic patients with normal pulmonary function and unr...
متن کاملAchieving Symptom Control in Patients with Moderate Asthma
Disease severity in asthma can be classified as mild, moderate or severe based upon the frequency of symptoms or the severity of airflow obstruction. This review will focus on the treatment of youths greater than 12 years of age and adults with moderate persistent asthma. Moderate asthmatics may have daily symptoms that cause some limitation with normal daily activities and require use of a res...
متن کاملInhaled corticosteroids versus long-acting beta(2)-agonists for chronic obstructive pulmonary disease.
BACKGROUND Long-acting beta(2)-agonists and inhaled corticosteroids can be used as maintenance therapy by patients with moderate to severe chronic obstructive pulmonary disease. These interventions are often taken together in a combination inhaler. However, the relative added value of the two individual components is unclear. OBJECTIVES To determine the relative effects of inhaled corticoster...
متن کاملScientific rationale for inhaled combination therapy with long-acting beta2-agonists and corticosteroids.
The addition of an inhaled long-acting beta2-agonist (LABA) to an inhaled corticosteroid (ICS) gives optimal control of asthma in most patients and two fixed combination inhalers (salmeterol/fluticasone and formoterol/budesonide) are increasingly used as a convenient controller in patients with persistent asthma. There is a strong scientific rationale for the combination of these two drug class...
متن کاملManagement of asthma in the elderly patient
A significant number of older asthmatics, more often than in previous ages, have poorly controlled asthma, leading to increased morbidity and mortality. On the other hand, current guidelines suggest that most asthmatics can obtain achievement and maintenance of disease control and do not include sections specific to the management of asthma in the elderly so that it is more evident the contrast...
